Resource Request From

Please use the form to the right to request a new app or educational license to use with your students.

The following process will be used when a request is made for any new educational resource.

Once a submission is made, our Curriculum, Technology, and Special Education Team will review the request.
If there is need of follow up, a supervisor will contact the person submitting the form directly. If all information is completed accurately, the resource will be reviewed at the next Educational Resource Committee meeting. This committee meets after school on the 2nd Tuesday of every month.

The make up of the Educational Resource Committee will vary each month based on the grade levels and content areas of the requested resource but will consist of district and building admin, faculty, and support staff.

Supervisor Approval Required

All new educational apps or licenses must be approved by a supervisor (Kyle, Kris, Julie, Dan, or Kim) prior to being submitted for review by the committee. Please provide the name of the supervisor who approved this resource being submitted to the committee.

Additional Information

Please provide any additional information (e.g, cost, budget information) that you think will help the committee decide if this resource should be added to our inventory. Also, please provide the committee with some detail on what this app does that other district apps or licenses don’t do.
